Health and Wellness
At Northwest Christian University, the health and well-being of our students is of the utmost importance. The Office of Student Life collects immunization records in compliance with Oregon law and offers assistance to students seeking medical services in the community.
Immunizations
Oregon law requires students born on or after January 1, 1957 who are enrolled in 9 or more credit hours at Northwest Christian University to provide proof of two measles vaccines or meet one of the legal exemptions. Adequate proof is written documentation of the month and year of each dose of the measles. In accordance with Oregon law, the Office of Student Life collects and reviews submitted immunization documentation. You can download and use the Immunization Form below and review the Immunization Policy as well. The Office of the Registrar will be notified of students who are not in compliance with Oregon law. Registration to attend classes will not be permitted for students who fail to complete their documentation.
