Housing Forms

The following lists all the housing forms for students. Please read the description and click on the appropriate links.

University Room & Board Agreement: This form must be filled out by all incoming and returning students who want to live on campus.

New Student Profile: This form is to be filled out by all incoming students. This will help us in matching students with compatible roommates.

Immunization Form: This form must be filled out by all incoming students. It is required that all students be up to date with immunizations to live on campus.

Housing Exemption: This form must be filled out by students who wish to live off campus. Please follow the directions carefully and fill out the form completely. Incomplete petition forms will be returned. (This form must be completed every year).

Please print the applicable forms, fill them out completely and return them to the Office of Student Life with your $100 housing despoit.

Please return your completed paperwork by May 1 for priority housing placement. Students should receive notification and contact information for your roommate by mid-July. If you have any questions or concerns, please feel free to call the Office of Student Life at 541-684-7345.
